Now you can connect to your health care information at your convenience. You will have access to:
- Your lab results
- Your visit history
- Your hospital discharge instructions
- Your health summary
There are two ways to request access.
Visit our Release of Information Office on the first floor of the Hospital. The staff will assist you with enrolling in the portal. You will need to show your driver’s license and provide them with information about you—your Date of Birth and e-mail address. You will receive an e-mail with a link to the Portal, onetime user name, and password.
Self-enrollment is also available. Requirements include a medical record (M) number including all nine digits and a valid e-mail address that matches the e-mail address recorded at the time of registration. Click on Register for an account, then click Sign Up.

After enrollment your health information can be accessed through an app such as those used on a smartphone. Depending on the chosen app, you may be able to access information such as your medications, allergies, or lab results. If you are interested in using an app, please call the Release of Information office at 410-392-7090 or e-mail email@example.com.
