Union Hospital’s 2012 Annual Appeal
This year's annual campaign, being launched at the end of this month, will raise funds for a new program at Union Hospital -The Right Care in The Right Place. This program consists of three key components:

Palliative Care focus on providing patients with specialized medical care to relieve the pain and stress of a serious illness and provide the best possible quality of life for both the patient and their family.

Physician home visits connect health care professionals with an aging population experiencing physical mobility issues, patients needing help to insure adherence with complex prescription drug regimens, and those with limited transportation choices.

Patient navigation saves lives through more timely diagnosis and treatment of serious illness, eliminating barriers to care and connect patients with available health care services. Patient Navigators are compassionate, effective guides who bridge gaps in care to help patients, their caregivers, and their families.
Individually, the three programs address specific needs of our patients and others in the greater community. Together, they establish a continuum of care that creates a health safety net for the most seriously ill and vulnerable residents of Cecil County.
