Group Life & AD&D

Basic life coverage equal to one times your annual salary.  Employer paid benefit.

Supplemental Life Insurance

Additional private portable term life insurance. Maximum up to 5 x annual salary not to exceed $500,000. Spouse and Child coverage also available.

Short Term Disability

Salary continuation that pays 60% of your base 
salary for up to 52 weeks from the date of disability. Eligible after completing twelve months of continuous employment. Employer Paid benefit.
 

Long Term Disbaility

Insurance that pays 60% of your base salary 
Continues while disabled or until age 65. Employer Paid. Eligible after completing 52 weeks of STD. Employer Paid benefit.
