Medical & Prescription Plan Options

Choice of Premier or Basic Plans, Both are Blue Cross Blue Shield PPO plans. Premier plan is a low or no deductible plan. Basic plan is a high deductible plan. Prescription carrier is Express Scripts, www.expressscripts.com.

Dental

Delta Dental Premier PPO Plan – Two cleanings per year at 100%. Basic services at 80% and Major services at 50%.

Vision

United HealthCare Vision, Annual benefit that is included in your coverage when enrolling in medical coverage. Must be enrolled in medical to receive vision, can not enroll in Vision without medical coverage.

Flexible Spending Plans

Medical and Dependent Care Flexible Spending
(Discovery Benefits) accounts available. Pre-tax dollars used to cover medically eligible and daycare expenses.
